Clinical Station
The second station is a clinical scenario of up to ten minutes covering investigations, diagnosis and management, with a communication mark taken across it and a one-minute handover to close.
Format and marks below are the 2026 position. NHS England's Physician Specialty Recruitment Office confirmed in July 2026 that the 2027 process will be “largely unchanged”, and will publish the 2027 detail in autumn 2026.
Preparation
- Work to a structure you can hold under pressure: assessment, differential, investigations, management, escalation.
- Say what you would do and why, at the level of a competent F2 starting IMT.
- Practise the handover as its own skill — one minute, structured, and no padding.
- The clinical area is weighted 1.2 and the communication mark 1.6, so how you say it matters more than the raw content.
Top Tips
- Talk through your reasoning rather than jumping to an answer.
- Name when you would escalate and to whom.
- Use SBAR or an equivalent for the handover and stick to it.
